About Our Rehab & Nursing Locations  

As part of Michigan’s largest health care system, our long-term care programs provide access to award-winning health care. We have eight rehab and nursing centers where our long-term care programs are located. All of these facilities is designed to help residents live a healthy and active life to the greatest level possible.  
  
Our team works with the person served, their families or support systems, to develop an individualized treatment plan that addresses each person's goals and preferences. Our rehabilitation programs serve people with traumatic injuries, medical diagnosis, wound care, cancer rehab, and cardiac rehabilitation through our cardiovascular transitions program.  


We are able to safely manage medically complex issues such as: 

  • Wound care
  • IV medications 
  • Enteral feeding ('tube feeding') 
  • TPN (Total Parenteral Nutrition) 
  • LVAD (Left Ventricular Assist Device) 
  • Tracheotomy care
