The Rehabilitation & Nursing Center – Cedar was designed with the word “home” in mind. Conveniently located near downtown Grand Rapids, this brand-new 100,000 square-foot community home has easy on and off access to I-196 and bus lines. Newly built in 2024, this home has an onsite dialysis den, art room, art gallery and four spacious interior courtyards proving easy-to-access outdoor space for rest and renewal.
There are 120 licensed skilled nursing beds and 5 licensed acute care inpatient beds. The skilled nursing rooms are private and semi-private with their own window, restroom and shower, modern furnishings and new technology.

Inpatient Subacute Rehabilitation and Long-Term Care
The Rehab and Nursing Center – Leffingwell was designed with healing in mind. The center offers 80 private rooms with beautiful nature views. In addition to a dedicated interdisciplinary team, physical, occupational, recreation, and speech therapists will provide patients with a customized therapy program designed to meet individual rehabilitation needs. Our expert team of therapists will use the latest technology and techniques to maximize independence and mobility to help prepare patients to return home. We are able to provide care for post-acute rehab patients. The facility is specifically designed for the needs of our patients and is set in an environment designed to promote peaceful recovery.
Newly built in 2023, this 77,800-square-foot rehab and nursing center provides the community with a state-of-the-art rehabilitation facility and offers short-term, goal-oriented rehabilitation.

Located on a beautiful campus, Corewell Health Rehabilitation & Nursing Center - Commons Dearborn supports each individual resident and their families—including those in advanced and later stages of Alzheimer’s and dementia. Medical assistance includes an emphasis on rehabilitation, recovery, and comfort in a home-like setting. Our highly qualified therapy specialists and staff offer friendly encouragement and support to help each person with meeting and ideally, exceeding their goals while keeping their quality of life on track.
The majority of people, who utilize our rehabilitation services following a medical procedure and hospitalization, are able to reach their goals and return to their homes within 20 days.
The facility has 196 certified beds (100 Medicare / short stay and 96 Medicare/Medicaid) and offers robust Clinical Services which include but not limited to, wound care, orthopedic and geriatric specialists, a dedicated memory care unit, speech and language pathology, physical therapy, and occupational therapy.
The sub-acute floor is a 59 bed Medicare unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including doctors, social work, dieticians, PT/OT, hospice, and spiritual support.
The Chronic Care floor is a 60 bed Medicare/Rehabilitation and long-term care unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents.
The Dementia Care floor is a 60 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care.
The 111-bed facility sits on Hansen Care Park’s beautiful, wooded property with private (33) and semi-private (39) rooms, spacious showers and restroom facilities, nutritious, country-style dining, beauty salon and barber shop services, walking paths, state of the art rehabilitation areas for those who need physical, occupational or speech therapy and cozy places to relax throughout. We provide care for individuals who may require a variety of medical services including Short-term Rehabilitation, Long-term Care 24 Hour Skilled Nursing, Hospice and Respite Care, Restorative Care and Certified Wound Care.
Our inpatient acute rehabilitation program connected to United Hospital is located in the center of Greenville.
